Revisión conceptual del sistema ejecutivo y su estudio en el niño con trastorno por déficit de atención e hiperactividad

EXECUTIVE SYSTEM: A CONCEPTUAL REVIEW AND ITS STUDY IN CHILDREN WITH ATTENTION-DEFICIT HYPERACTIVITY DISORDER Summary. Objectives. To review the different concepts and definitions of the executive system(ES) and its relationship with attention and working memory. To apply these concepts to the theories about executive dysfunction in children with attentiondeficit hyperactivity disorder (ADHD). Development. A functional and terminological definition about ES is made considering previous theories and mainly based on the theory of the control of the action. The concepts of working memory andattention are defined and their relationship with the ES is analysed. The evidence of executive dysfunction in children with ADHD is described and recent hypothesis on the nature of the neuropsychological key deficit of the disorder are discussed. Conclusions. There is a lack of an universally accepted theory about the executive system function, although there is an agreement between different authors inconsidering that it involves the cognitive processes needed for complex, novel and aimed actions. Attention is a function that is different from the ES, but it is required for the ES functioning. Working memory is one of the most important components of the ES, but it does not account for the whole concept of ES. Although the characteristic pattern of ES dysfunction in children with ADHD is stillunknown, there are some recent evidences showing that they have specific difficulties in behavioral inhibition. INTRODUCCIÓN La investigación de las funciones ejecutivas cerebrales ha despertado, en los últimos años, un creciente interés. Sonabundantes los trabajos que estudian sus alteraciones en distintas enfermedades en el adulto, como la esquizofrenia [1], la enfermedad de Alzheimer [2,3], la corea de Huntington [4] y la enfermedad de Parkinson [5]. Pero es, quizás, superior el número de referencias acerca del desarrollo y disfunción de dicho sistema en el niño [6-8], especialmente en el trastorno por déficit de atención e hiperactividad(TDAH) [9-15]. Las denominaciones función ejecutiva y sistema ejecutivo se han obtenido por traducción de las expresiones anglosajonas executive function y executive system. En castellano pueden dar lugar a confusión si se toma en cuenta la acepción de la palabra ‘ejecutivo’ como el que ejecuta o hace una cosa, en lugar de persona que desempeña un cargo directivo en una empresa. Los términosfunción supervisora o función directiva serían, probablemente, más exactos en castellano, ya que nos estamos refiriendo a una función que realiza tareas de planificación y control de otros sistemas, es decir, tareas directivas y de supervisión. No obstante, en este trabajo se van a emplear los términos función ejecutiva (FE) y sistema ejecutivo (SE), pues es así como se ha generalizado su denominaciónen castellano.
